Below are links to forms commonly needed by students and parents.

Automatic Withdrawal Form - Tuition & other Fees	Use this form to set up an automatic withdrawal from your bank to pay for tuition and other fees that appear on your monthly statement.
Automatic Withdrawal Form - Annual Fund Donations	Use this form to set up an automatic withdrawal from your bank to make monthly donations to the Annual Fund.
Automatic Withdrawal Form - Legacy Campaign Donations	Use this form to set up an automatic withdrawal from your bank to make monthly donations to the Legacy Campaign.
Emergency Medical Release Form (2008/2009)	One to be completed for every students prior to the start of the school year.
Parent Volunteer Form	To be completed by parents who would like to volunteer their time at Geneva. It is used as a means to match volunteers to the needs at the school.
Authorization for Pick-up	To be completed by every family prior to the start of the school year.
Aftercare Information and Registration	To be completed by every family who anticipates using Aftercare.
Field Trip Driver Agreement	To be completed by parents who will drive on field trips
Computer Usage - Parental Consent and Student Cooperation Agreement	Dialectic and Rhetoric students who wish to use a computer at school are required to sign this form before being given a user name and password to the school network.
Student Driver Registration Form	To be completed by students in 11th and 12th grade who drive and park at school.
Add/Drop Form	Used by rhetoric students when requesting to drop and/or add a class.
Consortium Registration Form	Fill out and return to Michael Miller to register for Geneva Consortium.
Pre-arranged Absence Form	To be filled out and returned to the Deb Scholz at least a week prior to the date of absence.
Community Service Hours Student Report	Students in grades 9 - 12 should use this form to record all their community service hours. After completing the form, please hand it to the Assistant to the Dean of Students.
