Below is a list of medical
documents that we must have on file for each student
enrolled at Geneva. Please read the explanation under
each item carefully, as this information is important
to the health and safety of your child. Do not hesitate
to contact our School Nurse, Lou Jones, RN, if you have
any questions. Please know that the purpose of this
information is to serve as a reference, not to give
you information to memorize; therefore, please continue
to refer to this information as necessary. Please also
refer to The
Geneva School Medication Policy for further information.
Please turn in any applicable forms to us by August
1. For new students to Geneva, some of the records should
come from your child’s current school. We will
send those Requests for Records over the summer, and
you will be contacted later in the summer if we do not
receive all of the necessary records from your child’s
school. Please note that your child may not
attend Geneva until we have all required forms.
Forms that are required are as follows:
New TGS students: If your child has
been in a Florida school, this should come in current
school records. However, if your child is starting at
TGS in Pre-K, K, or 7th grade, he/she will need to have
the updated immunizations and the new form turned in
to TGS. If moving from out of state, you will have a
Florida physician or the Department of Health fill out
the form, and then you will turn it in to TGS. You will
need to bring with you any documentation of previous
immunizations to the physician or the DOH.
New TGS students: All new students coming from another Florida school should
have already had a physical performed upon their initial entry into the Florida
school. If this is the case, the form should come in your child’s previous
school records. All new students coming from outside the state of Florida are
required to have a new physical performed and to turn the form in to TGS, unless
they have had a physical within the year prior to enrollment at TGS.
New TGS students: If your child has
been in a school, this should come in current school
records. If this is your child’s first school,
you are responsible for turning in a copy to us.
Should your child have a life-threatening allergy or
chronic medical condition that could affect his/her
daily activities at school, I would like to schedule
a meeting with you and your child before or during the
first week of school. Together we will formulate an
Individualized Health Plan for your student. If you
are interested in doing this, please call our receptionist
at (407) 332-6363 to schedule a meeting with me after
August 7th. For those children with Epi-pen prescriptions,
an Authorization
for Administration of Emergency Medication for Severe
Allergic Reaction must be completed and signed by
the parent AND physician. Two sets of emergency medications
are to be supplied by the parent - one to remain in
the classroom with the child and one in the clinic.
If your child has asthma and carries an inhaler to be
self-administered, state law requires that an Authorization
for Asthma Medication Administration at School be
completed and signed by the parent AND the physician.
Please note that the FHSAA athletic physical evaluation forms, required for
all 6th-12th grade students participating in a sport, are separate from and
in addition to the Department of Health forms. See the Athletics page for more
