Athletic Forms

FHSAA Pre-Participation Physical Evaluation (EL2)Required for all 6th–12th grade athletes. Parts 1 and 2 are to be completed by the student and their parents. Part 3 is to be completed by a physician. This form MUST be completed and turned in to the athletic office or to your coach before a student may participate (including practice) in any middle school, JV or varsity sport. This form is valid for 365 calendar days from the date of the evaluation as written on page 2.
FHSAA Consent and Release from Liability Certificate (EL3)Required for all 6th–12th grade athletes. This form is valid for 365 calendar days from the date of the most recent signature.
Affidavit of Compliance with the Policies on Athletic Recruiting & Non-Traditional Student ParticipationThis form must be completed by any new Geneva student athlete who enrolls at Geneva in 9th grade or above, regardless of whether that occurs during the school year or during the summer period between school years, including youth exchange, international, and immigrant students. Homeschool students participating with Geneva for the first time, 9th grade or above, must also submit this form. Please note, this form must be notarized.
FHSAA Homeschooler Eligibility Form (EL7)This form must be completed once each school year by all homeschoolers wishing to participate on a TGS team. Note: This form has changed, so please read the instructions carefully. An athlete may not practice prior to final approval from the FHSAA.
TGS Homeschool Athletic Participation Consent and Release Form (K4–5th Grade)This form must be completed for every homeschool student in K4–5th grade who intends to participate with Geneva youth sports. This form is valid through the end of the school year.
